Paper #5 Rubric
General Instructions: Students will determine whether or not their selected health program was effective or ineffective. What would you change or do differently next time? What advice would you give to the next person planning their health program?
The paper should be no less than one page and no more than two pages (not including the title page and references). Paper should be completed in 12-point font, double spaced with 1-inch margins in black ink with references if applicable.
In addition to including the above, make sure that you also mention
 Paragraph 1: Determine whether or not your program was effective or ineffective from papers 1-4 so far. “In order to ensure that ______ program was effective, an evaluation process will occur”… out of the two types in CH. 10, which program evaluation type will you use? Systemic or summative… formative or informal. State which type you will use and why you chose to use that type over the other.
 Paragraph 2: Determine what you would change about the planning process of your program- “If the program was granted additional funding, the program planners could have incorporated…” or “Next time, the team of program planners will…”
 Paragraph 3: What advice would you give to the next person planning their specific health program- What did you struggle most with? What was easier for you to complete within the planning process?
 Paragraph 4: How will your data be collected in order to successfully evaluate your program? – Quantitative or qualitative? Which method will you use to collect feedback? Don’t forget to mention the reason why you chose to evaluate your program and technique used for evaluating
Be innovative, think outside of the box and find something that you are passionate about to develop a program on- remember ethics when you consider your program requirements.
Grades will be determined based on all of the above details in your paper WITH explanations, APA formatting, grammar and overall organization. When citing information, in-text APA style citations MUST be included to avoid plagiarism. Each paper should be no less than one page and no more than two pages (not including the title page and references). Paper are to be completed in 12-point font, double-spaced with 1-inch margins in black ink with references.
Things to avoid: Beginning sentences with “And” or “But”, run on sentences without a comma, incomplete sentences, spelling errors, using conjunctions like “we’re” or “I’m” and abbreviations, (be sure to spell out each word), including periods at the end of each sentence. Check and proofread for grammar sentences or wording that may be confusing.

Imagine that you are the HIT Director for a healthcare facility and you are tasked with leading the facility in converting their office to electronic health records. For this Assignment, you will design and construct a Microsoft PowerPoint® presentation that highlights the planning and implementation process when converting to electronic health records. You will present this PowerPoint to other stakeholders in the HIT Department. Please include the following topics in your presentation:

Minimum Submission Requirements
This Assessment should be a Microsoft PowerPoint presentation (approximately 15-20 slides), and include title and reference slides.
Respond to the questions in a thorough manner, providing specific examples of concepts, topics, definitions, and other elements asked for in the questions. Your submission should be highly organized, logical, and focused.
Your submission must be written in Standard English and demonstrate exceptional content, organization, style, and grammar and mechanics.
Your submission should provide a clearly established and sustained viewpoint and purpose.
Your writing should be well ordered, logical and unified, as well as original and insightful.
A separate slide at the end of your submission should contain a list of references, in APA format. Use your textbook, the Library, and the internet for research.
Be sure to cite both in-text and reference list citations where appropriate and reference all sources. Your sources and content should follow proper APA citation style. Review the writing resources for APA formatting and citation found in Academic Tools. Additional writing resources can be found within the Academic Success Center.
Your submission should:
include a title slide;
be appropriately spaced;
use a neutral-style font (such as Ariel, Times New Roman, etc.);
include correct citations;
be written in Standard English with no spelling or punctuation errors; and
include correct references on the reference slide.
If work submitted for this competency assessment does not meet the minimum submission requirements, it will be returned without being scored.

For this assessment, prepare a two-page memorandum outlining the differences between the new reimbursement models and prior, traditional models for stakeholders.
Support your assertions in the memo with at least three academic sources. This may require you to do additional independent research. You may wish to consult the Health Care Administration Undergraduate Library Research Guide before you begin any additional research.
The assignment has 4 parts.
Part 1

This part should be half to one page long.
Part 4
• Explain quality concerns affecting reimbursement given a specific patient scenario.
• Specifically address the recent problematic patient case from the Vila Health: Investigating a Readmission scenario.
• Briefly discuss how the care provided would be reimbursed under prior models versus reimbursement under newer models, based on your assertions in Part 3 of your memo.

Health disparities continue to create challenges in population health. Certain factors contribute to the likelihood of health disparities. It is important that as leaders within population health, you understand these factors and what can be done to help reverse health disparity trends. PLEASE READ ALL INSTRUCTIONS CAREFULLY.
Look for 9+ sources representing a range of positions in the three categories, Support, Counterargument, and Alternative, on this topic. Make sure that these sources are all published within the last 5 years so that they give an up-to-date view of this contemporary issue. These three categories display the 360 degree range of alternative views on this topic, and one or more of them will at least be the basis for your upcoming Argument.
III. Cite each of these sources MLA style, but annotate them by summarizing their central points and answering the following annotation questions:
A. What puzzles, interests, and/or intrigues you about this source?
B. What is the research question your source is attempting to answer? Just as your Annotated Bibliography and this project begins with a guiding research question, so do your sources. What is it the author wants to answer in his or her project represented by this article?
C. Did the source answer its research question totally, in part, or not at all?

Take one paragraph of 4-8 sentences to answer these questions for each of your 9+ sources.
The 4 Annotation questions I have provided here are the standard set that could apply to just about any source you would ever encounter. However, the responses to those questions largely depend on the category in which the source is located in your project. For instance, a Support source is backing up your basic position, so it should be as persuasive as possible and answer a research question as efficiently as possible. By contrast, a Counterargument source should not be persuasive when it comes to your position, and it should not be the ultimate answer to any research question. Your Annotations of these sources may change where they fit in these categories, so have an open mind as you weigh the arguments in all these sources. One of the biggest acts of revision for an Annotated Bibliography is a reassignment of the use and categories of these sources in this evolving project.
IV. Find at least three sources to back up a position that you support and summarize the factual background they provide. These should be quantitative number and research facts. These are listed in a category labeled Support.
V. What are the major opposing positions? Cite at least 3+ sources that represent that side. Summarize the facts they provide in a category labeled Counterargument.
VI. What are sources that represent other positions rather than pro-con (generally, combinations and hybrids of the support and opposition positions)? What are previous attempts to solve this problem that have been argued for and/or tried? What can you learn from failures in the past to solve this ongoing problem? Keep a module journal of learning about health policy and the role of policy in the US health system. The journal should include your notes from the two criteria listed:
1. Watch the PBS Evening New Hour twice per Module (strive for a pattern of one Friday and another weekday of the same Module). Start with Module 2 for a total of 2 viewings per Module.
2. Read the weekend edition of the Wall Street Journal or The New York Times for a total of 4 Sundays (once per Module starting with Module 2). Healthcare managers must deal with three components of healthcare delivery: Access to care, quality of care, and the cost of care. With the move to a value-based care model from fee-for-service, there is a stronger connection between quality of care and payment. Organizations that accredit healthcare facilities, such as The Joint Commission, focus on quality standards. These organizations may also hold what is called deemed status. This means that the Centers for Medicare and Medicaid Services(CMS) will accept accreditation of a healthcare facility by an accrediting organization such as The Joint Commission as meeting the Conditions of Participation (CoPs). Healthcare organizations that meet the CoPs are eligible for reimbursement for the care of Medicare and Medicaid beneficiaries. Thus, the standards for accreditation include those that align with the CoPs. Managers must ensure that their organization meets these standards.
Prepare a PowerPoint presentation of 12–15 slides, not including the title and reference slides, with 150–300 words of speaker’s notes. Review the CMS Conditions of Participation for the type of healthcare organization that you chose in Week 1 and that you have used throughout this course. Include the following in your presentation:

One of the physicians on staff is telling you about a study he is doing on his patients. He hopes to have the study published and to receive a grant to expand his research. He offers to hire you to help him with the data collection during your off hours. You accept and begin helping him.
You start collecting data. When you access the file the next day, you realize he has removed a number of patients from the study. You suspect they were removed intentionally; however, you cannot be sure. You put your suspicions aside and proceed with your work and add more patients to the database. A few days later, more patients have been removed from the database.
After some investigation, you discover the patients who were removed were sicker and/or had a poor outcome.
